To email an absence form to the CSYO manager, please complete and submit the following form.
You must enter all fields and a valid email address in order to submit the form. A confirmation will appear on the screen after the form is successfully submitted.
Please only submit one absence per form. DO NOT list mulitiple dates! CONCERTS AND DRESS REHEARSALS are MANDATORY and do not count as part of the allowed absences in a season. If you have an unavoidable conflict with either a dress rehearsal or a concert date DO NOT submit an email absence form. You MUST contact Mrs. Rydel directly at 704-972-2003, ext. 231. The manager must know about the conflict at least 3 weeks prior to the conflict date.
